Year 9 School Leaver Booster E-Consent Information

The deadline for e-consent forms to be completed for the Year 9 School Leaver Booster immunisations has been extended to 23rd March, due to the date change. If you would like your child to receive this immunisation, please see the instructions to consent below. The school leaver booster immunsations will take place on Tuesday 29th March.
